Program Application Information
The Let’s Talk Offline application must be downloaded and signed by a parent or guardian for participants ages 10–17
Participants ages 18–19 may download and sign their application independently.
Completed applications can be submitted by scanning and emailing them to us. If you are unable to scan your application, you may call and arrange for pickup.
